Wellbutrin is a different product built on the same molecule. The Wellbutrin SR label indicates it for major depressive disorder. The Wellbutrin XL label adds a second approved use, preventing seasonal major depressive episodes in people diagnosed with seasonal affective disorder.

Bupropion Weight Loss Is a Reported Effect, Not an Approved Use
The Wellbutrin SR label does report weight change. In placebo-controlled trials, 14 percent of subjects taking 300 mg per day and 19 percent taking 400 mg per day lost more than 5 pounds, compared with 6 percent on placebo.
Read the framing carefully, because it is the opposite of a weight-loss claim. The label raises that weight-reducing potential as something a prescriber should consider when weight loss is already a presenting sign of depression. It sits under adverse reactions: a change to watch, not a benefit to pursue.
Prescribing Wellbutrin with weight loss as the goal would be an off-label decision. Off-label prescribing is legal and sometimes appropriate, but it is a judgement a licensed prescriber makes with a specific person in front of them, not a conclusion a reader reaches alone.
What Naltrexone Does on Its Own
Naltrexone is an opioid antagonist, and by itself it is not a weight-management drug. As extended-release injectable naltrexone it is approved for treating alcohol dependence in people able to abstain before starting, and for preventing relapse to opioid dependence after detoxification.
So the naltrexone bupropion pairing takes two ingredients with unrelated approved uses and combines them for a third purpose.
Why the Combination Is Theorised to Matter
The Contrave label describes its two components acting on separate brain regions involved in food intake: the hypothalamus, which regulates appetite, and the mesolimbic dopamine circuit, which handles reward. Nonclinical work found the two together increased the firing rate of hypothalamic POMC neurons and reduced food intake in mice.
The label is candid about the limits of that story, stating the exact neurochemical effects leading to weight loss are not fully understood. The theory is that the pairing does something neither ingredient does alone, which is why the individual drugs are not substitutes for it.

The Label Says Wellbutrin and Naltrexone Bupropion Cannot Be Combined
This is the part most articles skip, and the most important thing here. The Contrave label lists other bupropion-containing products as an outright contraindication, naming Wellbutrin, Wellbutrin SR, Wellbutrin XL, Aplenzin and Zyban directly.
Taking Wellbutrin alongside Contrave is not a stronger version of the same treatment. It is stacking bupropion on bupropion, and the label treats that as a combination to avoid, not a dose to titrate.
The risk driving it is seizure. Bupropion lowers the seizure threshold, which is why the label also contraindicates Contrave in seizure disorder or a history of seizures, in bulimia or anorexia nervosa, and during abrupt discontinuation of alcohol, benzodiazepines, barbiturates or antiepileptic drugs. Uncontrolled hypertension, chronic opioid use, recent MAOI use and known hypersensitivity are contraindications too.
Adding, swapping or stopping either medicine is a prescriber decision, every time.

These products are also not built to the same arithmetic. A Contrave tablet contains 8 mg of naltrexone hydrochloride with 90 mg of bupropion hydrochloride. Wellbutrin SR comes as 100 mg, 150 mg and 200 mg tablets, Wellbutrin XL as 150 mg and 300 mg.

The Boxed Warning They Share
Because Contrave contains bupropion, it carries a boxed warning for suicidal thoughts and behaviors. Antidepressants increased the risk of suicidal thinking and behavior in children, adolescents and young adults in short-term trials. Those trials showed no increased risk in people over 24, and reduced risk in people aged 65 and older.
The label directs that patients of any age starting Contrave be monitored closely for worsening mood and emerging suicidal thoughts. Contrave is not approved for children or adolescents, and not approved for treating depression or any other psychiatric disorder.
Wellbutrin carries the same antidepressant class warning. One more reason nobody taking either drug should adjust anything without their prescriber.
